Where information is submitted on behalf of a student, the person submitting the form confirms that he or she is the student’s parent, legal guardian, or an authorized person with the right to provide the student’s information and make decisions relating to registration, communication, class arrangement, and educational services.

若资料是代表学生提交，则提交者确认其为该学生的家长、法定监护人，或已获得合法授权，有权提供学生资料并就报名、沟通、课程安排及教育服务相关事项作出决定。
